Good answer, Caitlin L! You would be surprised how much bacteria you can have on your hands and under your nails.
~Make a conscious effort to avoid touching your face during the day.
~Use a gentle cleanser, toner, and a light-weight moisturizer (try to avoid oil based). I know it seems counter-intuitive to use a moisturizer, but in fact the skin produces a large amount of oil when it is dry (as a defense).
~Try following the wash with a cool splash of water (it helps to close pores), and occasionally use a masque to deep cleanse.
~Definitely try to drink more fresh water
~If you use make-up, try to find a light-weight natural make-up.
~Above all, try not to be too rough on your skin. Again, skin produces oils in defense, so when you scrub too vigorously you may aggravate the situation.

For me, there have been a couple things that have worked, but you will need to go to your doctor for a prescription. I assume you have already tried the standard creams that they sell at the drugstore.
The first is Stievamycin Gel (I think that is how it's spelled). It's pretty strong and did a good job at clearing up the acne on my back.
The second is takning Clindamycin. This is an antibiotic I started taking 2+ years ago and it has done a GREAT job at keeping my skin clear.

Follow a good skin care regime for oily skin. Drink lots of water (minimum 6-8 glasses a day); eat fruits and vegetables. Avoid fried foods and chocolates. Constantly touching your face will make the skin worse, therefore avoid that too.

If you drink any milk, stop right now. The iodine in milk causes acne sometimes. The most hardcore pill the doctor might suggest is called accutane. It works pretty well even though it supposedly has like 20 side effects. They are rare. Quit drinking milk.

GO TO YOUR LOCAL DOC (GP).
and don't waste your time or money with any of the cleansers, toners, moisturisers etc. they are usually expensive and disappointing!
your doctor will prescribe a tablet that is right for you, it will probably be on you have to take every morning and night after food. Minomycin is a common prescription.
