Most people don't do it, because they're not sure if it's even allowed in Islam to make an oath by the Holy Book but I've seen a couple of people do it and it seems quite simple.
All you really have to do is be clean (eg. make Wudu) and place one hand on the Qur'an and say your oath, whatever it may be.
Some people also say "Wallahi" but that's a more casual way of saying it. It's like saying, "I swear I......." in English.
Hope that helps. :)
In response to Tony T, it is actually AGINST Islam for a girl NOT to inheirit property, money, etc. becuase Islam goes completely against sexism. I'm not sure where you heard about girls not being allowed to get an education because that is completely untrue. There are Islamic school all over North America which are based on Islamic principles and both boys and GIRLS go to them and recieve the SAME amount of education. And Muslim girls are allowed to go out in public with thier faces uncovered. Some women just choose not to and in some very RARE cases (just like in every other religion) they have abusive husbands that MAKE them wear a niqab.
I know this because I am a Muslim girl and I'm as normal as any other girl in North America. I'm pretty sure I know more about my own religion than anyone from the media does. What they show you on TV are extremist cases that don't even apply to most people.

North America is a land where any one can practice his or her religious beliefs in the freedom which is not afforded this privilege in most Countries. So; all people lay-off the Muslims if they are not bothering you. I am a Christian and for Christians to insult another person's belief system is sheer arrogance mingled with attitudes of holier-than-thou ! Radical Muslims - Terrorists have nothing to do with most Muslims; so why do we behave like WE are spiritual terrorists here, in our land of FREEDOM ???

Not making it in the first place. There's nothing Holy about the Quran. Try the Holy Bible, it is the only truth on this earth. Jesus, the only one that can save you and I from Sin.

say
"There is no God but God, and Mohammad was his Messanger"
[2.136] Say: We believe in God and (in) that which had been revealed to us, and (in) that which was revealed to Abraham and Ishmael and Isaac and Jacob and the tribes, and (in) that which was given to Moses and Jesus, and (in) that which was given to the prophets from their Lord, we do not make any distinction between any of them, and to Him do we submit.

This country and its laws are based on Christian principles. If someone wishes to do a Muslim thing, then they should immigrate to a Muslim country. In those countries, they cut off the hands of petty thieves. What if they wanted that to be adopted by the US also since it is that way in Muslim countries. Should the laws of this country be rearranged to allow that. What about not allowing girls to own property or get an education or go out in public with their faces uncovered???? Where does it stop?

This is a Christian tradition, Muslims don't need the Qur'an, your word is enough(e.g. I will do such and such).
