It's definitely ok for the images in your portfolio to be vertical or horizontal. In fact, some images almost demand a horizontal orientation. Just make sure that your portfolio itself makes it easy for a person to change perspective on the page if necessary.

I think it depends on the photo. If it is a really good photo and you think it will help your portfolio then add it, just put it with a couple more maybe and it wont look out of place. As long as it adds something about your work then why not? I don't think there is a rule that says they all have to be vertical.

Mix it up a little....surely some pictureshave more of a calm feeling about them...put those horizontal. However, if you're trying to represent growth, action, or are striving toward a goal, having all your pics vertical surely could contribute to a common theme.

In my opinion it would be essential in your portfolio to show your flexibility as a photographer. So to my mind it would be essential to have both portrait and landscape modes on display.

Whatever looks best. Just try to keep them arranged in a visually pleasing way. You are the artist.

They should be as they are taken... however it is best to keep all of one kind together so the book does not have to be repeatedly turned to view the images.
