Your nerve is probably exposed. Im surprised you could let it go this long. You are right, you will need it extracted at this point - but don't wait, this could turn in to an abscess ( if it hasn't already) in which case you will need oral surgery to clean out the mess. OUCH!

I really feel for you. I know what that pain is like. If it's already rotten, the best thing to do is go have it extracted. Believe me, I wanted to kiss the dentist when I had mine extracted! Until then, take aspirin and use some OTC products like Orajel. Also, you can brush your teeth and use some mouthwash to kill any bacteria that might worsen the pain. The pain will only get worse, not better no matter what you do so you need to get to a dentist quickly!

I found that things like "oragel" made my toothache pain worse but I got reliefe from putting red pepper seeds around the tooth. Capsicun, the "hot" compound in peppers, stimulates the release of the body's own pain killing substances.

Load up on Advil.
Will need about 3 tabs four times a day.
Drink lots of water with the advil.
Advil is a decent pain reliever and will also relieve inflammation.
Good luck.
You need a dentist.

This works. If the pain is on the your ride side run your thumb along the right side of your jaw bone until you feel a slight indentation. (left side if the pain is there) Push up into that little notch and massage it and the pain will lessen. Its a accupressure point.
