Contact the Office of Vital Statistics in the state she was born in. They will charge a fee, and some may ask for information that the birthmother would know (hospital, date of birth, attending physician are some examples). These offices are usually in the state capital. Also, most states websites will help with this information. Good Luck.

No matter which state you live in, you need to contact the State Office of Vital Statistics. Depending on your state, the fee will vary. Most of them will require you to send in or fill out an application form stating your relationship (mother), a signature, and a valid photo ID for verification purposes to prevent identity theft.
If you require additional information, go to the Center for Disease Control (CDC) website below, which lists the procedure you need to go through. Just choose your state from the list, and it will tell you where to go for Birth, Death, Marriage, or Divorce Certificates.
