Sounds like something vascular, but if you have had cat and mri scan I wouldtry physiotherapy, this could be a traped nerve, or if not the physio can manipulate the atctual issue and correct it.

Depends on how old you are. If you're (ahem) of later years it's probably what I have; exactly the same symptoms caused by trapped nerves due to degeneration of the neck.
If the pain and so on disappear when you've been lying down for several hours, this is likely; the spine stretches slightly.
If you can take it, co-drydamol works wonders. I can't, so I have regular cortisone injections into the joints. These are magic!
An X-ray should show this, though, usually better than a scan will.

What i would like to know is were these test all ordered by the same md.? if so then i would suggest getting a second and maybe even a 3rd opinion.
This sounds like a very serious condition and if it were me, i think that i would probably seek many many opinions of doctors. Especially if this has been bothering you for 18 months.
have you seen a neurologist?

The best thing I can suggest is to see a craniosacral therapist. It's such a gentle treatment it's not going to damage you but it's powerful enough to make a difference.
Can't hurt to try.
Good luck
