Sounds like what I have.
I have a focusing problem, probably caused by too much time at the computer. I technically have 20/20 vision, but sometimes I can't see very well because my eyes don't focus properly.
Go to the optomitrist and have them check.
I have glasses, but they aren't always that helpful. It seems to help if I rest my eyes frequently and use eye drops a couple times a day.

Sometimes people who work too much using their eyes tend to get very tired eyesight which might be fixed with having glasses like 2 weeks but you should really consult with a doctor

Probably more of a respiratory issue. Try deliberately deep breathing throughout the day. Do you smoke? You might even have an ear infection. Or diabetes. Or a tumor. Go to the doctor.

I first noticed I needed glasses when the TV guide magazine became blurred. If things are clear maybe you are just reading in poor light too much.

My extended sits at the computer made me get glasses and I have 20/20 fission. They say you should get up and walk away every twenty minutes from the computer screen.

Possibly, you should get an eye exam. There are medical reasons for the symptoms you describe so you should also see your doctor..;

yes, if your vision is blurry, you can't make out words close or far from you.
no if you are just having blackout/dizzy spells.
that may be diabetes or poor nutrition

You need to see a general practitioner and he/she will refer you to a proper specialist. It might be your eyes, it might be something else. You need to have it checked out.
