Whenever you hear a word you are not familiar with, write it as it sounds to you.Pronounce the word to a friend to explain it to you,you might have the wrong spelling but the same sound and that clue could help you get the meaning and the spelling right. You can then use such words in your English ,either spoken or written.A good way to get new vocabulary.Get yourself a good grammar book to work on your tenses. Some times books for kids and early teenagers are a lot helpful because the authors do not want to confuse a young reader and so they (authors) construct very good sentences. If you go for "adult" novels, you will get confused because the authors think that their audience or readers are matured adults who understand the English language very well.Pick new words or sentences that interest you from books you read and use them in your everyday language. That is a good way to help you with the written and spoken English. However, avoid extremely big words.
When you write on a topic (essay or comprehension),keep your sentences short or brief to avoid errors in your tenses. If you are in the US,don't depend a lot on the spoken English it won't help you in the written part. There are a lot of slangs and verbs disagreement in the American English,so get a good grammar book to help you to write very good English.if you are in the US,go stop speaking what you speak but if you are in UK or a former British colony, don't inject American English into what you are taught at school. If you do,you are likely to fail in English.

Try taking a class - there are plenty of classes out there that teach English. Many colleges offer them, and there are also classes called "English as a second language" that you may find helpful. Just google them (or use Yahoo search, whichever you prefer).
