It has a masculine ending (the o) which means its refering to a male. So it means extremely handsome or extremely good looking or extremely beautiful. We seldom say a male is beautiful in English, so I would choose either handsome or good looking. Also the illisimo is an ending which raises the degree of comparison. It raises handsome to extremely handsome. This is why I say it means either extremely handsome or extremely good looking.
