Make up a book of memories for her..photos of her family, poems etc. Create perfumed pages with images and also maybe burn a cd with all her favourite music from the past. It annoys me when people say she wont remember things. Maybe not, but you can re=create things for her to enjoy. Her mind is somewhere else, try to spark off something for her to relate to. Its never too late to make someone content and happy. Good luck

What era is the lady 'living' in? If she believes it's the 1940s or 1950s or that she's a child then buy a present from that era.
Past Times do some great items.

My granny is always happy to get anything, but she really likes nice things that aren't necessary but make life more pleasant. Posh chocolates, a pretty pot plant, a fancy room spray (she can't use perfume on herself) are always good for her.

A decent pair of shoes! My neighbour has Altziemers and he walks and walks for miles. Or maybe a pedicure to look after those tootsies.

Something that she can appreciate with her senses, tactile or olfactory, such as a soft blanket, fresh flowers (fragrant roses, carnations or lilies), perfume or bath talc.

a perfume called 4711. my gran loved it.she had altziemers. every time see smelt it. it brought back memories from a different time.we still do not get it,but it worked for her. she smiled every time she looked at,smelt it and wore it. you can get it boots now.
