In addition to the ADD and ADHD suggestions above, the symptoms you describe are also found with depression, stress (or even post-traumatic stress disorder), nutritional deficiencies, allergies, anxiety and sleep deprivation. People with these symptoms are often misdiagnosed with ADD or ADHD.
Perhaps take a personal inventory of these possibilities. They can all happen at once, and symbiotically feed off of one another in a discouraging cycle.
Take heart, some lifestyle changes can be very beneficial, with exercise being a great start, as it reduces stress and anxiety, promotes a far more restful sleep (you may have a sleep disorder without realizing it), and promotes endorphines in your system that combat depression. As well, nutrition is important as well, of course. Lack of certain fatty acids (GLA's ect.) as well as omega 3, omega 6 and omega 9 (among many others)can also give rise to the symptoms you describe. Allergies to certain chemicals (and our society is FULL of them) and foods, etc. can also create these symptoms.
Reseach these issues thoroughly, through credible resources, such as a local health centre or clinic. A naturopath (or other reputable natural healthcare provider) is a great way to go (the initial fees can be a bit to swallow), as they can look at all of these aspects as unique to you. As well, they won't hand you a quick diagnosis and put you on a pill that masks symptoms rather than exploring all of the possible options that create what you describe above. Not to knock MD's, but it must be said that more "grey" areas are covered by naturopaths and the like who will investigate these things far more thoroughly (the initial appointment usually takes about an hour to an hour and a half to discuss your concerns whereas a physician has only about 10-15 min at the most). The naturopath will want to and have the time to look at the root causes first, because sometimes the remedy is often a nutritional, or lifestyle related. As well, they can look at stress management, sleep hygene, anxiety and depression, test for allergies, etc. and take a direct approach to these things first before attempting to diagnose you with something.
As well, meditation and yoga and personal growth with healthy, loving and supportive like minded people is always nuturing as well. Even if you do have ADD or ADHD, the symptoms can be brought under more tolerable control with the measures described above. As well, those with ADD or ADHD may have their own particular challenges, but are often of highly superior intelligence in some aspects, and it is the symptoms which may affect other types of intelligence and approaches to learning.

I highly recommend taking up meditation, even if you just try it for 10 minutes a day, to try to focus your thoughts on NOTHING. Try to clear your mind of all thought. It actually takes a great deal of focus and practise to master this, but can help you get centred in your life.

Not a disorder... you just need to cultivate attention. Meditate... even if it takes 5 years to build up to 5 minutes.

ADD. I've got it. I take concerta for it. Go see your doctor, because it sounds like you definitely have ADD or ADHD (Attention deficit disorder) or (Attention Deficit Hyperactive Disorder)

Quite often this is consistent with ADD. If you think you might have this see your doc for a referral to a testing facilty and get this checked out. My son has it.
