I can provide you with my experiences:
I usually try to talk to myself in the English words and sentences I know when I'm alone, then I began to "produce" English sentences when I talk in English. Eventually I began to talk fluently in English, and my grammar has improved a lot by listening more and reading more in English.
To improve your listening, try not to read the subtitles when seeing a movie; just listen to the lines. You can also look for some radio stations in English online.
Singing English songs would help you a lot too. I recommand songs in the musical The Sound of Music. They are very easy to remember for English learners.
One of my teachers in school suggest that English learners should read a lot, and the vocabulary in the first few pages of the book should not be over 5%. You can write a summary or comment after reading them, and ask someone to correct it for you.
All in all, the key is to make yourself surrounded by English and use English as much as possible. 加油!你做得到的!

Teach Yourself English Books have CDs and are very good for learning pronounciation. also there are software programs for learning English, watching BBC news and reading BBC news online. Speak as much as you can with a native speaker of English. You can also learn new vocabulary by reading.
