You should get it out through surgery. It should not be too hard. Dont leave it in the tissue too long.
As for lead poisoning, maybe , after it breaks down chemically and leaches through his bloodstream . WHy wait until that happens?

that is something you ask your doctor when he/she is removing the bullet from your butt.
