It is because of a intense emotional reaction. When the vagus nerve is stimulated, due to fear, anxiety, or physical stimulation, the heart rate is decreased. When the heart rate slows, blood pressure may drop enough to cause fainting. Sometimes the body's process to regulate blood pressure cannot respond quickly enough.

Some people get caught up in the thought of the pain they know is involved when blood is drawn or spilled. It tells the brain there is intense pain involved and the brain reacts as if it is experiencing this pain. I knwo the first time I was in the OR watching a surgery while in resp school the sight of the blood combined with the smell of the room caused me to faint. 20 years later I no longer have this reaction. Your brain learns to disassociate the sight with the experience.

When I get my blood drawn, it isn't the sight of the blood that makes me faint - it's the actual feeling itself of the blood being taken from me that causes me to faint.
