By restraining her, this will make matters worse by frightening her.
Use Vaseline on her bottom and tip of the suppository.
Show her the procedure perhaps with a doll - promise her a reward as soon as procedure completed and give her praise.
Explain to her why she has to have the suppository. What is the reason? Is she constipated? or suffering from nausea?

If she is a normal four year old, she is TOO OLD to need to take suppositories. You should definitely use other delivery methods. If, for some unfathomable reason she MUST take a medication this way, have her do it herself, and simply observe to make sure she holds it in. I would highly recommend NOT holding down a four year old to insert (and keep!) an object in her rectum. If you do this, she will learn to push it out as soon as you're not watching; plus she will feel that her private places are not her own but rather are able to be "owned" by adults.

Ok you need to have someone help to hold her. The best way is for her to be on all fours, and gently insert it. Another way is if she lies across your lap. If it is medically necessary, as I assume it is, try asking her pediatrician for a different route of administration. Good luck

This may sound barbaric--but have her lay on her left side. stradle the top part of her--to secure her-- then bend right leg to reach the rectum to insert the suppository You may need assistance if this cannot be done. Good luck (She will probably expell it as soon as you insert it)

Oh man did you just give me a horrible flashback...just don't give it to her. (It's a good thing I am not her parent right, lol.) My only advice to you if she is not comfortable with the person who is going to be restraining her, then wait 'til someone she is comfortable around can do it. Have you tried to do it while she is sleeping?

Ask her to bend over and grasp her ankles with her hands. Give her ice cream after or a treat

WOW! How barbaric! I hope this does not cause her emotional problems later on in life. I suggest waiting until she is asleep.
