As well as speaking from your heart and saying you are sorry to hear of her death, say something in the card (or enclose a letter) with some memories you have of how special she was to you, or something you particuarly liked about her or if you will miss anything in particular about her.
I know it's sometimes difficult to know what to say but saying nothing and ignoring someone when they are bereaved is the worst thing to do.

I honestly don't know... three weeks ago an old friend of mine suddenly lost is wife and I just don't know what to say. So I haven't even dared to contact him since hearing the news...

One has to point out to the aggrieved person that he (she) did not deserve the bereavement, though one has to face the inevitability of destiny (or unpredictability of events). These moments make one introspect on the impermanence of worldly interests and helps one to develop equanimity. The sorrow will be healed by passage of time and the loss shall appear to be a permanent treasure of love in One's memory.

I would say something like "There is nothing really right to say at a moment like this, but I am sorry for your loosing (name)." Then say something nice about her, if you knew her.
After, be sure to say that he is welcome to call anytime.
He will need to know people will still accept him now that he is alone.
