Get it up right under the heat a few inches away and cook it about 10 minutes per side. It really depends on how hot your broiler is but 10 minutes per side is good. In a restaurant kitchen with 700 degree overs it is more like three minutes per side.
Here is a big secret: the very BEST way to cook a steak is to cook it in a frying pan on the burner at high heat. It sears it on the outside and keeps it tender on the inside. Try it and you will never broil a steak again. About 5 minutes per side.

In a normal broiler in an electric oven with the rack on the top I cook 2 min per side for rare, 3 min per side for medium rare, 4 min per side for medium, 5 min per side for med well, and 6 min per side for well. If you cook it any more than that you will have shoe leather. One minute will not cook it enough even at highest heat, so I don't know what all those people are doing voting thumbs up for the 1 minute.
Some people like steak to moo when they eat it!

10 minutes per side sounds like way too much. I do 4-5 min per side for Med. Rare and my steak is not even on the top rack rung.
