Only if you are leaving the country.
Beginning January 23, 2007, ALL persons, including U.S. citizens, traveling by air between the United States and Canada, Mexico, Central and South America, the Caribbean, and Bermuda will be required to present a valid passport, Air NEXUS card, or U.S. Coast Guard Merchant Mariner Document, or an Alien Registration Card, Form I-551, if applicable. This is in addition to all previous requirements.

Depends if you fly international or domestic. All international travel requries a passport no matter what time of year or place unless your passport country is in the EU.
If you have an EU passport, you can travel to most other EU countries without a passport!

Depends on where you go.
If it's a domestic flight, you don't need it. A driver's liscence will work.
If it's an international flight, you will definitely need it. You might also need a visa. It's best to have at least two types of ID with you, so bring a passport and a driver's liscense. It couldn't hurt.
