Is he potty-trained? Or maybe in the potty-training process? That could cause him to actually hold his poop, which leads to constipation, which leads to painful bowel movements when it just has to come out. Or he could be holding it because he's becoming more aware of his body and what comes out of it and he's a little afraid of BM's or grossed out by them. Just follow the other advice people put on here, don't overload him with fiber or anything, but do make sure he is getting enough. Make sure he drinks plenty of fluids, not juice all day, give him water too. And a glass of apple juice should help his BM's along nicely. Encourage him not to hold the poop. If you are potty-training and he's resistant, or feels more comfortable pooping in a diaper, don't push the issue, he just might not be ready to be completely potty-trained. Another tip, make sure he's getting enough age-appropriate exercise, like riding his tricycle, chasing him around the house. You can also lie him on his back and bicycle and stretch and bend his legs, that always helped when my daughter couldn't poop. If he doesn't like to be still for you to do that, sing a silly song or turn it into a game. Good Luck.

No, this is not normal. I would highly suggest getting a second opinion. Mean while you can increase the fiber in his diet to help him go more often. Juicey Juice always make my kids poop. try giving him a couple glasses of that as well. Constant constipation is not normal and it is painful. It could be as simple as something in his diet but it could also be a complication with his intestines. I would see a different doctor. good luck and Merry Christmas.
PediaSure is a nutritional supplement, it replaces vitamins, etc that the child may not be getting, it doesn't really help regulate his bowel movements.

Sounds like he is doing it to himself by trying to hold it in my brother used to do this becasue he hated going #2 at school, only going once a week is not at all normal my 2 year old goes every other day sometimes 3 days.
Try heating a mug of hot water and letting bay leaf soak in it, then make the water into hot chocolate and let him drink it, my mom always did this with my brother and by the next morning he was going like a champ.

Make sure he drinks pleanty of liquids. Take his weight and divide by 2. This is how many ounces of water he should have a day. Also, try giving him apple sauce or something with a good amount of fiber.
