if there were 4 people not on a bus, and 3 didnt get off, how many people would be on it?
answer: 3 people. the "4 people" were never on the bus in the first place, and the "3 people" didnt even get off the bus.

3 because they didn't get off. And the original four weren't ever on it.
Or we could change it to "If there were 4 people on a bus and 3 people get off how many people are on it? " Then I would have to say 1.

3 or 4, depending on how you interpret it. The first double negative means there were 4 to start with. 3 didn't get off. That means either one got off or no one got off, so 3 or 4 are still on the bus (i.e. "not not on it").
