No matter which way you look at it you can't say it outfront. You could be extrmely direct but unless you guys have been flirting and stuff it won't work. Show more interest in him first flirt, call him cute, make jokes. then ask him and tell him. it will make it a lot easier and make it a lot harder for him to refuse becuase all those old feelings will be ignited. First of all... DO NOT sound desperate.
If it was your fault, Just say that you have had time to think about where YOU caused the relationship to go wrong, and that you regret the decisions you have made. then boost up his ego, a man always needs his girl to stroke his ego!
Tip: List all the great things he did for you, how wonderful he made you feel... how attracted you are to him, and how no one will Never understand you or treat you the way he did.
If it was his fault:
It should be relativly easy just say that you forgive him and that you hope he has made some changes... then the magic words..
"Baby I want to have what we had before, lets try to make this work, I promise to give in 50 % if you can do the same, I love you ! lets get back together!!
