We Catholics take it to the Church to settle the dispute.

15: "If your brother sins against you, go and tell him his fault, between you and him alone. If he listens to you, you have gained your brother.
16: But if he does not listen, take one or two others along with you, that every word may be confirmed by the evidence of two or three witnesses.
17: If he refuses to listen to them, TELL IT TO THE CHURCH; and if he refuses to listen even to the church, let him be to you as a Gentile and a tax collector.

Very few Protestants I know actually follow the instruction from Matthew 18 that you cite (although I think we should). Sadly, what usually happens is what we call a church split. That's one reason why there are so many small Protestant churches in any given town. If we Christians truly followed the example of Christ and the teachings of the Bible, the world would be a different place. I think it was G. K Chesterton who said, Christianity has not been tried and found wanting; it has been found difficult and not tried.
