doing it for some debate thing and need to know some examples or sites where i can find more info

Nuclear plants have a better safety record than any industry in the US. (there have been no deaths and only rare injuries, not severe) They have triple checks on things, not just double.
All radioactive materials are kept in lead or concrete containment. The waste materials are turned into a type of glass and buried in deep caves in the south where there is little fear of earth quakes and it is below the water table. Transportation is well gaurded.

Well if you are looking at the potential problems they are different for fission rather than fusion reactors. I have a vested interest here as I have a new patent "Emergency Roofing and Barrier System". The main failure of fission reactor buildings is in their roofs. if you search the IPO website you will find my patent details - enuf send! without a complete reddish of fission reactors and possibly a loss of efficiency this seems the best safety improvement possible immediately

here are some of the examples of 'safety' i have come across.
1. not knowing there was a breach of the secondary shielding in a cable riser.
2. allowing the reactor basement, where you store low level contaminated articles in plastic bags, to flood on high tides.
3. shutting down the wrong reactor after a false fire alarm.
4.storing fuel rods in a corridor
5.not warning anyone you are doing pressure testing on the co2 system and then having to rush in and get them out of the reactor hall when it leaked.
6.security not letting the fire brigade on site when your co2 bellows catch on fire because of unsupervised welding.

The key to nuclear safety is Time/Distance/Shielding.

Minimize the "Time" you are exposed.

Maximize the "Distance" between you and the source.

Maximize the amount of "Shielding" between you and the source.
