Balls in your court. I would give him a chance since he has been through re-hab but I would take it slow.

I heard somewhere that when u do a 12 step programme in rehab, one of the steps instructs you to apologize to the ones you hurt when u were high/stoned/drunk or on any kind of substance. Maybe that is what's step he is on now.
Regardless of any reason as to why he did what he did, (and your ex-best friend should be ashamed of herself also), at this point in time IT'S ALL ABOUT YOU.
If you feel that you want to be with this man then by all means give it another shot, but make sure, absolutely sure, that you take your time, take it REAL SLOW, watch over yourself carefully, so you will not get hurt again.
Also. Many addictions are very hard to get rid off, and it's a lifetime battle, understand that you might be taking a lot on. Make sure you take this into consideration if you re-start this relationship.
I wish you the best of luck,
Irene

Let him finish working on himself by getting (and staying) clean and sober, after that if you're still interested in him (chances are that you won't), then take it from there. =0
