Body language is vastly important. Try to work on any nervous tics you may have, so that you can come across as clean and confident when you ask. If you can do that, it oftentimes won't really matter how you ask, but saying "will you go out with me" may come across as a little forced or stiff.
I usually ask, "are you doing anything this weekend?" or "would you like to do something this weekend?".
Or, if you had something specific in mind, say it. "Would you like to go have some coffee?"
Really, remember that you want her to remember you. That means being imaginitive. Sometimes it will work in your favor and sometimes it will work against you, but that impession is often all you have to go on and you must make it count.

You ask to take them out some place nice. E.g. If she likes fish- 'This great new sushi bar has opened on the fancy side of town, fancy going after work for a tasty treat?' Sheeesh! Will you be asking her if you can be her boyfriend next...!?
If your only 14, who knows? U could try that way, directly.. But u need to make yourself seem promising (If she agrees to do 'XYZ' with you, what will she get out of it?)

I think the most common phraseology is something to the effect of: "You wanna do something sometime?" Not terribly direct but it gets the point across without sounding too committed.

Only if they are in Jr. High...
Try asking if she wants to go get a cup of coffee sometime, of if she wants to catch a movie with you this weekend. If you ask her to "go out with you" it will be a little akward. If the case is you are actually in Jr. High...then go for it

Dani what about in the mid west? we have creativity as well its just that everyone around here likes the direct approach more or less.

Some people find out what they like to do. Then when they ask, "How would you like to see (the movie she is dieing to see) or "How would you like to here (band she wants to here). It give another point in your favor for her to say yes.

depends on where you live. In the west of the US, yes they usually ask in creative ways. In the east, peope tend to be quite unoriginal.

I wouldn't. Just mix it in to your conversation. While talking, "So listen, let's go to _______(specific place) Friday to ______.
she says, "OK"
Then tell her what time you'll pick her up, and if you don't already have her number, ask for it, so you can call for directions before you go.
