Find a Quiet Place, not your room, not your home, like library or dining room actually works for me. other thing is ironically headphone with music does help to keep mind on one thing, it helps you memorize things for final exams.

music helps me too. But I know what distracts me: internet, tv and friends. So, I have to force myself out of my room, away from my TV and away from friends. Leave your phone in your room, go to the library or a coffee house and don't take a laptop with you. Once you are somewhere you aren't as comfortable as you might be in your room, you will have no other options but to look at those dreaded books.
Best of luck!

in my case, go to school, go to somewhere not so quiet, but very boring, like the conservatory or libray, then stay put there... until you have settled to that place, and can't think but to study, since you have nothing else to do!
does that headphones work? haven't tried it yet, but having that idea distracts me already...
