First of all -- make sure you understand *why* he smells of urine. Does he have a medical condition that prevents him from "holding it in", or is he just not practicing good hygiene. If it's a medical condition, make sure that he gets it taken care of, or starts wearing adequate protection.
If it's just a hygiene matter, you should speak with him in private, and tell him politely that others have noticed that he has a strange odor about him, and you wanted to point it out to him so that he wouldn't be embarrassed about it later on.

Your Grandfather either knows but can do nothing about it himself, or he is unaware of the smell. Old people often lose the sense of smell but develop incontinence to some degree! If he lives alone is he capable of looking after himself? If he has care from someone then perhaps you should talk to them first!
If the smell IS coming from the cats you will need to find a solution to suit all concerned. Your Grandfather might not even mind smelling of cats if he likes their company! Keeping the cats away from him would frustrate both cat and owner! Do me a big favour and respect your grandfather!

Try tactfully asking him if he needs help with his laundry because you have noticed that his clothes have an odor. He is probably having accidents and can't discern the smell himself so he doesn't realize that other people can. He may need to be reminded to put on fresh clothes every day or to change his clothes each time he wets them. Maybe he needs more clothes but for some reason, finances or just old fashioned thrift, can't get them. Probing questions can help uncover the real reasons and then you can figure out a solution.

Try to tell him in a caring way, don't be too harsh about it. Perhaps your Grandfather has some sort of bladder problem.
Tell him genlty about the smell, jtell him that he should perhaps see a professional about the problem if there is one. From there his personal hygeine is likely to improve. If he doesn't have a problem as such tell him that it is always important to maintain good personal hygeine, the smell of urine is probably one of the worst problems in relation to ones personal hygeine. It is off putting to say the least, you can't just sit back and say nothing at all.

You need to find a way to tell him so that he doesn't feel embarrassed. You might say something like..."Grandpa can I talk to you about something ? gently and softly say... grandpa , sometimes as we get older we tend to have bladder problems. sometimes when i'm by you i can smell something that kinda smells like urine. one of my friends grandpa had the same problem and now he changes his clothes every day so he doesn't have that problem now. can i or someone help you with making sure you have clean clothes to wear every day ? .....something like that might be less harsh then sayin' DAMN GRANDPA YOU SMELL LIKE P*SS !!! lots of people are that mean. obviously you care for your grandpa or you wouldn't have come here searching for the right words to say to him. It's really hard to give this kind of advise when you don't know the person in question. but if someone were to tell it to me like i just told you.....i think i would be thankful and happy that you cared enough to help. I really hope this helps. we're all gonna grow old someday. and when i do , I hope i have someone like you that loves and cares enough about me to tell me about a problem i may have like this. Buy him some strong-smelling cologne for Christmas, and ask him to spray some on to make you happy, then every time you see him, be like Gramps! how come you're not wearing that cologne..and hopefully he'll always wear it.

He is proably aware of that. when you get old parts of you body don't work so well and atlot of the toxins and waste seeps out through the skin instead of the liver and the kidneys
