Since he doesn't seem to care and he doesn't seem approachable to request that he check his muffler and get it repaired, then contact the noise abatement office in your area. Or contact motor vehicle dept and see if they can issue a ticket. You can also check with local police, since it's a noise complaint. Good luck.

You can always complain to the police, or to his landlord if it's a rental, or talk to him directly about it. But I would ask the following first:
Is he going to work? (probably)
Can he afford to fix it? (maybe not)
Maybe if you talk to him there is somewhere else he could park- or at least not leave it running a long time. My husband leaves a little before that every morning for work, and we had a car for a few months that had no muffler & we just COULD NOT fix it- not enough money- although we were really trying. The alternative was getting up the kids at that time and taking him to work with our other car (which we did, every day, for months before we got that car) but school had started and I didn't want to keep doing that to interrupt their sleep like that and wouldn't consider leaving them alone! He parked on the side furthest from neighbors & didn't warm it up (scraped the windows, started it and went)
Don't assume this guy doesn't care. We sure did. You of course, deserve not to be jerked out of bed at such an unreasonable hour though.

Get something to make white noise in your bedroom, a simple box fan will do, but you could also get a air purifier that will make enough noise. People sleep through trains once they get used to them. You really cannot do anything about your neighbor. Unless you want to offer to pay to get a new muffler for him. Maybe he doesn't have the money, and at least he is going to work. Try to let go of the anger and walk in his shoes for a moment. If you are able to do that, you may find that you start sleeping right through it.

You should call the police and report a noise violation. You can get a ticket for having your car stereo too loud so maybe this is similar. I'm sure the police will at least come by to tell the guy is truck is too loud, if not give him a ticket. My downstairs neighbor once called the police on me for playing my radio at 2 am. It may be up to him to fix his loud truck. Who knows maybe he doesn't think he is bothering anyone.

Visit him one evening when he is home and tell him that you heard him starting his truck this morning and ask him if there is a problem with his truck that you can help him with. He may not be aware of the problem that is causing you...just be polite and see what happens

Wait for him to go back in the house while the truck warms up....then move it down the street. He'll freak when he comes out and won't do it again.

Perhaps he is aware of the noise .....since he is up at 4:30 everyday.....he must be a responsible person with a job......but has nothing any better to drive . Maybe he goes to work to better himself. Quit bitching and go back to sleep.

You may be able to contact your local poilice and they can speak to him about getting a muffler and that the neighborhood has been complaining. You can stay annonymous.
Hes probly going to work, but if its THAT loud, he needs to do something about it.

That's his transportation, and there's not much you can do about it but get used to it. People who live near the tracks learn to tune out the train; you can start a routine when you hear your neighbor's truck. Get up, go to the bathroom, go back to bed until it's time for you to rise.
