Edvard Munch was showing how cruel the world has become. People scream for help, but nobody cares to offer assitance or compassion. The world has become a scary place to live in. Who's your friend? Who's your refuge? Notice the person in the painting is in great agony and suffering, however the couple walking on the street (portrayed at the top left of the painting) carry on walking and never bother to even look at the person who is screaming.

People of the world, wake up. Don't say that other people are nothing but strangers. If you can help, do it. What would you do if you're in someone else's terrible situation?

He just heard that Nick and Jessica broke up. Actually it is about everything that was going on at that time. War, genocide, etc.
