What's the difference between theses phrases?

1. ¿How are you?
2. ¿How do you do?

First of all, it wouldn't be English grammar with the spanish question marks.

But...

the first one can be either a polite greeting, or a sincere inquiry as to someone's well being.

The second one is a more formal greeting for someone you are meeting for the first time.

Formal and informal. Like in Spanish there is a direct way to ask an adult or someone your not friends with.

Formal and informal-usage of questions as well.

Unfortunately over time we forget common curtsy and use more laymen terminology when addressing someone even if it's a superior or someone we do not know.

Also it depends where you are in the states, people in the south use more terms with, 'sir and mama' then is used in the upper division of the US and east and west coast.

No idea, I blame TV.

1. is used in a more informal context, and people might actually expect an answer (although not necessarily a sincere one), depending on how well they know you.
2. Is used in formally greeting somebody at the first meeting, and the ONLY answer that is expected is "How do you do?" in return.

No much really. They are both used in more or less the same way but how do you do is more formal and likely to be reserved for the first time you met somebody.

How do you do is a polite greeting expressed when one meets another, including a stranger or newly introduced person.
How are you is, in my view, used when the other person is more familiar with you (family member/friend/colleague, etc.). This implies that you would like to know something more about his/her general welfare, something more than a 'how do you do'.

"How do you do?" is more of a formal greeting when you are meeting or introduced to a new person. "How are you?" is more of a friendly question for a friend or acquaintance to find out how they are what is going on in there life that sort of thing.

Most people say "How are you" upon seeing someone they already know. "How do you do" is used more at a first introduction.

Well, they basically mean the same thing when ask in polite conversation, but literally “how do you do” would be asking someone the “how do you do SOMETHING?”, no one would take that literally though.
