There is only 1 New York, because Americans like to give everything nicknames, like The Big Easy, Windy city, York is the original and it has been in England UK for over 1000 years,
York Minster is much older than the whole of the USA.
the Apple was given its name because it was a major port and everything came through it and forced it to expand and grow up at an alarming rate, it was said that it grew like an apple into the big metropolis it is today, They name it twice because you now have Uptown New York and Downtown New York.

I think it has something to do with the fact that New York State is known for its apples, and so the largest city is therefore known as "The BIG Apple."
