Go to the doctor's office and fill out a release of information form and they will give you a copy of your boy's records.

The good news is that you should have no problem getting a new copy of your son's immunizations!

The first thing I would do is contact his pediatrician. If you let them know that you lost his immunization record, the office staff should have no problems recreating a record for you. Doctors (at least the professional ones) maintain very careful medical records and these records are maintained the entire time your child is a patient and for a number of years afterwards. Your son's doctor refers to these records every time you bring him in for an appointment so that they are aware of his medical history - this includes all information on immunizations, illnesses he's had and any treatments prescribed, family medical history, developmental milestones, etc.

It should simply be a matter of the doctor (or his.her staff), flipping through your son's chart and recordig the dates he received his various shots (dTap, polio, HiB, Hep B, etc.)

If you got them all at his doctor's office, then they have a record. If not then some states have an immunization registry where they keep track of all the kids and what they have been vaccinated against. Your doctor should know about it if your state has one. Otherwise if you've followed the schedule I would honestly just write down what the schedule says. By 2 he should have had all the ones he needs for now.

Well not knowing where you are from, you might have to deal with it differently. In BC you can go to the health unit and ask for a new book. Your son's doctor will also have his immunizations on record. Just call his doctor and explain what happened. They will probably be glad to help you out and point you in the right direction.

Contact the place(s) that gave him the immunizations. If he received shots at the health department and the pediatritian's office you'll want to request the info from both.

I misplaced my daughters immunisation book though it didn't matter as i get sent a copy of her immunisation record every so often. You can go to the doctors and ask for an update on your child's immunisations im sure they will provide the information to you.

You should be able to obtain a copy from your family MD who gave the shots.

you can get another at the dr. office or health clinic where you took him to get his shots. just explain to them what happened and take the proper identification.
