There are two ways to dock a puppies tail: cutting and banding.
Cutting is done at 2-4 days of age. The tails is snipped off with sterilized scissors. If done properly, there will be only a small amount of blood. Apply Neosporin and some antibiotic/antiseptic powder to stop the blood and prevent infection.
Banding is done at 2-5 days old and is less painful. For this method a specially-made rubber band is fitted onto the puppy's tail. The rubber band quickly shuts off blood supply to the tip of the tail. The puppy feels mild pressure and then nothing. After about 3 days the tail end will fall off.
Both methods are safe when done by an experienced person. if you would like to dock your puppy's tail, have a veterinarian or experienced dog breeder show you how. I have included links to more information. The first has a video and the second has good pictures. If you watch the video, do the high-resolution one. The procedure is much easier to see.
I am guessing your puppy is much to old to have its tail docked. If this is the case, your only other option is to have your veterinarian remove it surgically under anesthesia. The reason the anesthesia is necessary is because docking and older pup is very painful.

i have no problem with the issue of tail docking, as my dogs all have docked tails and i am a doberman pinscher fancier, but my honest opinion is that you should let a vet do it or at least supervise if you want to learn. the equipment you need can be expensive and if things should go wrong, you won't have the training necessary to prevent tragedy. tail docking is done at SUCH young age, when the dogs are barely born and still so fragile, it's really best to leave it to professionals. most puppies won't even go home until their tails are healed. ears however, are another story. :)

For breeds where it is part of the "breed standard" to have a properly docked tail, the breeder should have a vet do it within the first 5 days a puppy is born. If your puppy is older and you don't have the proper experience in doing this (reading how on the net and actually doing it are not the same thing), then you should consult your vet about having it done properly with the proper anesthesia.

Please do not ever consider docking your own puppy's tail! I think it is a safe assumption to say you do not have the facilities, medical training, or medical experience to attempt this operation. Only a vet should ever undertake a surgery like tail docking, and even then, a very reputable and experienced one. If you try to dock his tail, their is a good chance he will bleed to death and die. There are a whole host of other complications as well. Tails are supposed to be docked in the first week of a pups life anyway, so he is probably too old for this surgery. Please don't do it!

You don't, a vet has to do it since it is serious surgery. If your puppy is old enough to be away from its mother, it's probably too old to have its tailed docked now.

You take it to the vet between 3-5 days old... I have been breeding dogs for the past 38 years, and I will not attempt it..too much can go wrong.
