medicines? if ur referring to the giving of medicines like tablets to patients, these can happen if the health workers are overworked/ over burdened and overtimed. but if ur referring to the practice of doctors of medicine, there are not that many mistakes that u think of. they spend a lot of years in medicine training.

there aren't as many as you think but mistakes happen we are all human most mistakes happen because the person doesn't pay attention to what they are doing sometimes if a person has been working a lot then they are more likely to make a mistake
