since a dd-214 s controls so much,including GI benefits I don't believe blank forms are available except through a DOD personnel dept. on a military base. Is it worth going to jail for?

If you asked me personally How you could do this, I would be checking up on you to make sure that you were not claiming awards or service time that was not valid.
I think it should be a federal crime with a minimum of 1 year hard time when someone claims ANY award or service time period that was not earned by the individual.
More time for higher awards such as bronze star or above....
So to answer your question...
You don't need one!

Unfortunately, a search of the DOD forms site shows this form is a controlled form. The only people that can download the form electronically are the personnel department custodians of the different services.

You can't, this is one of the most guarded of forms, and it is NOT available the public. You get one copy, and that is it.
