When you are approaching a door with a woman and the door opens INTO the building, so that you have to push it to open it, do I (1) Go ahead of the woman, holding the door open standing in the building or (2) stand in the door way, the woman squeezing by as I hold the door open with one arm or (3) let her push the door open I stand close behind her to hold it open with an arm while standing behind her?

One, definately. 2 feels crowded and weird for both of you and three feels creepy. I don't really want to feel pushed through a door by the guy looming behind me.
If it feels weird to push ahead, say something or she will get it when you stop and hold the door.
I may be a woman but it is just courtesy for me to hold a door for people behind me and I like it when others help me as well.
I don't understand the people who are making this a big deal. I hope you would hold a door for a guy as well.

Tell her "let me get that for you", then follow (1).

(2) is too awkward.

(3) is actually not doing anything yourself, but allowing her to open the door and you're just following her in before the door closes.

Wow you are a knight in shinning armor. If the door is enormous and heavy, it would be nice for you to go ahead of her, but if it's just a door let her push it herself and follow.

I tend to go first, your option 1. On the other hand, if the devil is in me and it is a narrow door option 2 is not to be sneezed at.
