Domestic Relations in your area can answer the questions about child support.
My knowledge is that regardless of name, both parents must provide child support.

Yes the court house is the right place to obtain the form to legally change your name. You may ask your county if you need the form signed by both parents to change the child's name.

Don't forget to change it with your credit card companies, credit bureaus, banks, drivers license (only have 30 days to do that), all registrations, and most of all IRS, and state plus local taxing offices. This can be very involved, as they need you to use their form, and you must provide the legal name changed form that is time stamped by the prothonetary's office in the court hourse.

Then make tons of copies because there are many who need it, I almost forgot utility companies, phone, cell phone and others.

GOD bless.

The name does not change Dna. No matter what her last name is ,if he is proven to be the father he has to provide for his child. If he refuses than you take him to court, and they will make him pay support for his child.
