There is no consistent relationship between atrophy (shrinking) of the brain and dementia. 'Mad cow' disease is rare and presents with a great deal more than simply dementia. Vascular dementia implies that 'poor circulation' caused the dementia.

Vascular dementia is dementia that is caused due to blood flow problems in the brain usually high blood pressure related, as compared to Alzheimer's related dementia in which the brain actually shrinks.

In Mad Cow disease the brain shrinkage is over a period of a few days as compared to dementia where the brain reduction is over the course of years.
