Your local post office, you can print the forms out beforehand if you go to http://usps.com/ they have directions on everything you need, ie. picture and how much it will cost you.

The post office has the forms. But you also need to get a "passport photo" which most commercial photographers are able to do for you relatively inexpensively.

I need to get me a new one myself....looks like my best bet is my county courthouse. If you need it "express", then you need to find the passport office in your nearest federal government office.
