It depends entirely upon the situation. If he is an old geezer and lives out in the desert and herds goats and camels and has 46 grand kids probably he's called a sheikh. A Sheikh can be the head of state such as the UAE.
The sufis also call their leaders sheikh and the normal villiage headman is also called sheikh. tribal leaders are called sheikh.
The leader of group prayer is called the Imam. However an official Imam of a mosque has clerical schooling and authroisation.
Anybody can lead the prayers . So the leader of the Muslims can be literally anybody of age . You'd call them by their name brother so and so. A lady could be the leader of other women if she were a scholar. She could be called sheikha , Alima, Amira or Sultana if royalty. On occaisions a Qadi- the judge.
The leader of a small study group or other religious group is called the Amir, Amira if a woman. Amir also means prince and can be a title of state in a caliphate.
The leader of a confederation of Muslim states is called the Caliph.
Haveing a fully developed legal system and military organisation they also have titles for people of rank in those organisations as well.
The whole notion that some guy just comes 'outta the blue and appoints himself the leader of the Muslims is a new innovation. No Muslim upstart can tell the Muslims of the entire world ""follow me, do as I say or else I will declare you to be an apostate and kill you"" It's likely that if we had a caliphate that guy would be hanging from a rope outside the gates of the palace. Sedition, Brigandage, Murder Banditry are illegal so those guys don't get any honorifics. Those are all capital crimes because they usurp the staes authority. Part of the problem with destroying the educational system of a people is that they no longer can produce leaders with any real qualifications in governance. Some are just now learning it. I don't know what you call the amateur leaders you know- like the famous destructive cult leaders. We do have legitimate leaders and for the most part they are pretty squared away. I love the sufi sheikhs. They are the leaders with hearts and minds that work in unison to serve mankind. They build orphanages feed refugees, rebuild destroyed homes ,teach people to read and practice medicine . They lead people into righteous liveing. Visit their sites on Dr Godlas' site.

If he is the prayer leader of a mosque then he is called a Imam and if he is a scholar of Islam then he is called a Mufti I hope this answer's your question

An Imam is the leader of a a mosque and a caliph is the leader of a country. Mullah is the leader of a tribe.

well that depends on what you mean by leader, if it the one who leads the prayers in a mosque it is Imam, or Shaikh, if it is one that leads the entire Muslims of one country interms of religion only it is Mufti , if it is the one ruling a Muslim country it is a king in arabic Malik "Saudi Arabia", if it the person ruling all the Muslim countries like old times he was called Khalifa.

BUT the nice, NORMAL ones are called "Imam's"
