I wish to find some one who can help me create a 7 day meal plan. I want each mean to provide me with the correct amount of vitamins and minerals as well as proper amount of fruit, vegetables, diary, meat and any thing else I am missing.

I pretty much want a cheat sheet so I can just buy the ingredients make my meals and not have to worry about what to make.

are there any people I can go to help me out with that? perhaps a nutritionists?

Yes, they are called Clinical Dietitians. They usually work in Hospitals with Patients. However some also work on an outpatient basis. They can sit down with you and help you with meal planning. If you are diabetic, you may want to visit with a Diabetic Educator. They are also Dietitians, but they specialize in educating people with Diabetes.
If you are unsure whom to contact. Call your local hospital and ask to speak with someone in the Dietary or Nutritional Education Dept. That should give you a head start.
There are some "Nutritionist" out there that received their "degree" from some other place than from a college/university. Ask them some questions before making an appointment. Some questions you may want to ask are:
1) How long have you been a Dietitian/Nutritionist? 2) Where did you receive your degree? 3) What kind of population do you usually deal with? 4) How often will I meet with you and what are your expectation of me? 5) How much does it cost? (some may charge a fee, some may not. it may depends on "Why you are seeking help").

A dietician/nutritionist can create a meal plan for you considering your current weight, desired/suggested weight, height, gender and age. They can also tell you how many calories a day you should consume so if you want to be more free in what you eat that you can just keep a food diary and track the calories each day.

A nutritionist is probably your answer. One word of advice is not to make each meal absolutely correct, just the average.
