To check if an individual is at risk of hypertension or hypo tension.
Namely to check and advise the patient the risk of high blood pressure will result in higher risk for getting stroke or heart disease.
For hypo tension, to make sure patients are not anaemic which make them faint easily or easy get giddy.
A nurse can advise on what food they can take, additional blood test to run.

Basically is to have a baseline data so that a nurse or doctor can further evaluate and educate the patient.

it depends on the patient and their level of understanding. use basic terms and analogies if necessary. the reason for taking the blood pressure is to help find out if the heart is pumping the blood at the best rates to help keep you healthy. if not you need to see a doctor for a good exam and tests.

It totally depends on the patient's level of understanding. Are they a child? A demented adult? If they are a normal person that is capable of understanding. I would simply tell them that the systolic is the pressure that is in their vasculature when their heart is pumping blood, and the diastolic is the pressure in the system when the heart is at rest.

Do people really have a problem with you doing that? I've never gone to a doctor who didn't take my blood pressure. Tell them it's to make sure they're not dying.. or better yet, when they ask, punch them in their face for being stupid! People are so weird.

Its is rare that a patient gets to the wards without understanding this.
But,if it occurred and I was asked that Q. I would answer something like"Dr has requested that your B.P. be taken every 4hrs(or what ever it is)because, he/she needs to assess if your B.P. is within normal range."That would be it.
As it is not ethical for a nurse to give a diagnoses, or information other than is absolutely necessary.
Any further Q.s I would tell them "I shall ask your Dr. to explain that to you,as soon as he/she arrives on the ward.

Blood pressure is a screening teat that can detect hypertension, heart disease, and many other conditions. As we age our BP tends to creep up and that is why they call it the silent killer. It doesn't always have symptoms. It can tell if you have kidney disease. It is a great tool for the Dr. to gauge your health and make sure he is taking good care of you. It tells him a lot. It's for their well being and continued good health. Whatever they need to hear to get it done and satisfy them.
