He is a civilian when in a civilian capacity. Most hope for this scenario, as they do not like to be reminded that the President is also a military figure, just like El Commandante, in Cuba. As Commander-In-Cheif, the President carries the effective rank of Five stars. He is saluted by ALL military personel and all of his orders are obeyed. Sorry. No dice. The President IS in the military, regardless of opinions. His word, and his word alone, as the highest ranked military member authorizes launch or use of nuclear arms.

Aside from that, any of the Cheifs of Staff from wartime years would be the highest ranked officers. General Collin Powell comes to mind, immediately, for his leadership in Operation Dessert Shield/Operation Dessert Storm. I suggest looking the Department of Defense website up for a list of all of those who have held Five Star status in the US Armed Forces.

the last person in the USA to have the highest rank was a 5 star general . 5 stars are no longer given.
at present the highest or most important position in the military is the Charmain of the joint chiefs of staff

Washington and Pershing. They were both appointed to the rank of six stars, however Washingtons rank was never worn by him, it was issued sometime after the Revolution, as I understand it!

General Washington, as in George Washington, our first president. I think the other one was General Pershing, although I'm not sure. They were both 5 star Generals, although George Washington wasn't given that distinction until long after his death.
