It's a bit of both; a person who is mentally sound already most likely won't become an anorexic because she wants to fit into what society seems to think is a normal weight..but someone that does already have a predisposition for mental illness may succumb to an eating disorder because they are in a situation where being skinny might be a plus.

However, there are other underlying problems with anorexics than just body image, but body image certain can jump start it.

In a way, yes. Fat is 'bad', and skinny is 'good' and glamorous. And all these kids, especially girls, try to model themselves after all thes super-thin models and stars. Ergo, eating disorders.

And on the other side of the coin are fast-food restaurants advertising super-sized portions of a meal that isn't healthy to begin with. And with people being on the go so much, these meals become tempting.

What we need to do is get a whole lot of reality checks and start cooking at home again.

No mental issues. It's a mental disease.
