Find a place you feel confortable to have a date, call her and ask if she had spare time next days. Take a free time of her and say "I'm doing (put somenthing you woud be doing that day here) this day, but I would like to see you, so I can free some of my time..." and just say you want to go on that place..
The key to bypass shy is keep practicing, you will certainly grow better each time you ask a girl out. Just think the "asking out" as a game you could risk somenthing to may win much more, and feel the fear wishing to beat it and win all.
Btw, at first, don't lose time thinking how you ask her, use this time to go and ask her out. Be confident and, after all, if she say no is the same thing as if you don't have asked her, you can't loose what you don't have. Hey! u sound like a very sensetive guy,thats great! i love shy guys they are the best anway in your case being shy is not going to get you anywhere because girls like guys who take control of things.The girl u like might like a shy guy or maybe not but either case when it comes to asking her out,the next big step,you have got to get your act staight and go for it!! i mean what is the worest she can say no and that it is not even half bad!! atleast you tried and that makes you a man! just walk up to her with flowers and tell her she is the best thing thet has ever happened to you and that you want to take her out then take her hand and lie a kiss softly. The female gender are VERY sensitive and know where us guys are coming from before we do!!! Just show her your best qualities. Avoid the bad ones-forever, if youcan! She will pick up on your likeness of her. Maybe she already does! DON'T verbally ask her out, dude!!! Let your actions do the talking. (Remember the third sentence). If you're not confident, you are going to make pretend you are.
If you don't look good, buy some nice clothes and cologne.
