Another co-worker and I are organizing our holiday party. We walked around the office telling everyone to pull a name and if it is their own to fold it and put it back in the bowl. One associtate pulled a name, my co-worker and I saw it say my name. She folds it back up and said it was hers. She pulled another name and said this one is ok. Should we call her on it?

No. Just chalk it up to worker who is not so nice. This has happened to me. The person who got me bought something cheap and cheesy. So, it may be to your advantage that another person selected your name. It's not worth calling her on it. Someone else said, "Don't sweat the small stuff."

Talk to your boss on a one-on-one manner. Tell him everything this lady is doing. Tell them you do your work the best you can and give it your all but this lady is making you seem like you don't. Say you don't have nothing against the lady but it's hard to work with her. Make out a plan to see if one day you can work alone or without the lady to show the boss that you are a good worker like she saw before. Explain to her in every detail of how this lady acts on her job. Maybe the boss doesn't realize she is slacking. If you have to find a way to take a picture of her with her feet on the desk doing absoultely nothing. Or have somebody else say something to the boss. You said you work with another guy who does the exact thing you do. Have him make a complaint along with yourself to show the boss you aren't the only one being affected by this and not the only one who see's her laziness. Hopefully eveyrthing works out. In the mean time I would say it's best to look for a job quite similar to the one you have now. That way you can rely on another job if this lady makes you more angry. It's not a good idea to let other people get the best of you but if you have to work under these circumnstances it's not right. Then if you possibly choose to quit and have another job you could explain to the boss that you are quiting and tell her the reasons why. Maybe then she will take into consideration that she really needs you for the business and tell you to stay while she replaces the admin asst.

No. She is not the first person to do that. I have done it myself if it is someone that is hard to buy for or for some other reason. Don't let yourself get all worked up over it. Like the saying says "Don't sweat the small stuff, and in the end it is all small stuff"

She already knows what she has done. To tell her that you are also aware of it will only cause friction. Leave it alone and be satisfied that you now know her true nature. Perhaps invite her to church or bible study. In other words, offer to help her but do not chastize her.

I wouldn't. It probably isn't worth causing a tense situation in the workplace. If it was supposed to be anonymous, you should not have seen it anyway. And, really, if it wasn't for the fact that you KNOW it was your name (and you therefore feel slighted), would you otherwise care?

you could but why even bother it's obvious that she has some sort of issue with you and if she's that petty then she was going to give a suck gift like pencils or something I just pray for her
