If you're in London you could try Homechoice (cable provider).
If you don't want (or can't get) cable, at the moment you have to START with BT, who will install the line and get it up and running. Once it's installed you can then move away from BT if you wish and choose to pay another company for your calls (and, nowadays, line rental too).
BT still look after the exchange and connection to your dwelling, so they always have to do the installation.

You would have to get a phoneline from either bt or a local cable company(there would only be one cable company in your area probably NTL/Telewest).
There are other companies you can go to to get a landline that gives you a BT line without going through BT. Some examples would be The post Ofice, Carphone Warehouse, Homecall, Tesco, Tiscali, Southern Electric/Swalec.
This would still be a BT line but the company you go with would handle the line for you.

BT is not the only company other than cable. Any company who can charge line rental can activate/ install a line.
The problem is that most companies do not want to pay the connection charge that BT Wholesale charge to switch on a phone line. That is why they tell people to call BT and get the line activated and then to call them and they will pay the £18 cancellation fee cos it is less than cost of doing it themselfs.
But they can do it, just tell them that if they want your business they will do the work and pay the money it costs for your business. If they cant be bothered to put the line on do you really want to be with them, when at the slightest hint of a problem they are going to send you back to BT.
Companies who can switch your line on are:
Bulldog, Talk Talk, Post Office, One Tel, Pipex. There is 5 but there are many more.
Hope this helps.
