The Carina which is located at the posterior and inferior of the trachea (where it divides into the left /right pulmonary bronchus) is the most sensitive part of the respiratory system and responsible for triggering the cough reflex.

The cough receptors are innervated by the vagus (10th cranial) nerve. Typically, the cough reflex is stimulated in the trachea and perilaryngeal spaces (area around the vocal cords) and is involuntary. It does not involve the brain (only the brainstem).
Voluntary coughing involves the 10th, 11th and 12th cranial nerves, which innervate the pharynx (throat). Additionally, because it is voluntary, it does involve the brain (parietal lobe).
