absolutely! If you had bad breath would you want your friend to tell you so could do something about it.

Be subtle to start with if you can!! Try offering them lots of chewing gum and mints... hopefully they'll take the hint! If not, you might have to tell them - but be tactful: in a place when its just you and them, and be as gentle as you can - it's going to be quite embarrasing for them but If you do it right, then they'll appreciate being told.

close friends or family- yes. Just try to be tactful. Try telling them how good your new dentist is, or how much you like the new toothpaste you're using (it doesn't have to be new at all) and they should get the hint.
I would not tell acquaintances or co-workers. If you're not close enough to hug them when you greet them without being slapped, I would not advice telling them.

I would want my friend to tell me so I would, it depends on how close your are to the person as to how tactful you need to be my b/f for instance i just flat out tell him his breath stinks, friends i say here you need one n give them gum...they usually laugh. if you don't really know them just offer mints or gum, if they refuse politely excuse yourself from the conversation.
