Something has caused her real stress..that is a symptom of some kind of trauma..either she has been punished severely, or been frightened by another dog, or even had a very bad dream.
Dogs pee where the scent is covered by the master's scent, when they are stressed..This way, they think no predator can find them, buy following their scent..

Many times if an animal who is trained starts going inside, there usually is a reason. She could have a urinary tract infection. Has anything else changed that would have upset her, are you gone more often, longer this time of year. has someone moved in or out that could have upset her system. I would take her to the vet and if no medical reason, talk to a trainer, see if they can help figure out what has upset her.

has there been any recent changes in the household? A new person or someone leave? Longer hours at work? If there have been any noticeable absences from yourself or someone else she is attached to it may be a behavioral problem. Call your vet and discuss the issue with them. They may want to do some test to make sure she doesn't have a UTI (Urinary Tract infection) or and other health issues. Good Luck!

yes. She's 11.5 years old. She's getting old and as dogs get old they start to become incontinent. It could also be signs of manyother illnesses. She could have a urinary tract infection and therefore is trying to pee somewhere she finds comforting cause it hurts, she could have cushings disease or diabetes or some other illness which causes an increase in urination. I would suggest taking her to the vet. At 11.5 years old, she is a senior dog so changes in behaviour may hallmark something more sinister than we'd like. Best case scenario is that you get her a check up and there's nothing wrong. Good luck!

Check the urine, is there any blood or crystals in it? Then she should be seen and a sample brought to the vet. Could be a urinary infection.
Is she spayed? If she's intact then it could be hormones and a heat coming up.
If there are no health reasons then you need to back up with your training and go back to the crate for awhile until the training is reinforced again.

Has anything in her life changed recently? Such as: a new pet brought into the family, a new child or human (boyfriend, etc.) brought into the family, or a move?
Have you taken her to the vet to check for a urinary tract infection or other bladder issue?
Have you scolded her badly while catching her do it on the floor or carpet?

when dogs act out, they usually do things like that to get our attention. boredom and depression, even a new addition to the household that might make your pet feel threatened will do it. first rule these things out before jumping to the conclusion that your dog is sick.

dogs never urinate in a place of sleep especially their own.
if they have an accident it is usually in a corner or some hidden place.
i have to agree - it could be an infection or disorder.
